IVSC Card Key Request Form

The Indian Valley Swim Club uses a card key security system for access during published pool hours. Pool members are granted two (2) card keys upon initial registration. Keys are deactivated at the end of the swim season and reactivated once dues payments are received at the beginning of the next swim season.

NEW MEMBER KEYS:
Please fill out the form below, and select New Member Key Request. Your dues payment will be verified with the club treasurer and new keys will be mailed to you.

LOST/DAMAGED KEY REPLACEMENTS:
Please fill out the form below, and send a check for $5.00/key (payable to Indian Valley Swim Club) to the following address. Your new keys will be mailed to you after payment is verified with the treasurer.

DEFECTIVE KEY REPLACEMENTS:
